I can reproduce this, but in a different fashion. The new notes in a midi region only get played on second playback. Also moved notes get played after 1st playback. On first playback the first notes (those on beat 1.0) get played VERY loud, making me push the panic button.
I don't exactly know what's the real problem, but I noticed this behavior. You can mail me, i'll try to reproduce these bugs.
This all was on ubuntu 10.10 amd64 with ./waf j3 using jackd2

This bug is still present under 7945 / openSUSE 11.3 32bit - only the first note gets played on the first playback of a MIDI track where the first note is at exactly zero:

Create a new MIDI track, set grid mode to Grid, grid units to Bars, push rewind to make sure you're at 0 then create the new region as near to 0 as you can. Then insert some notes with at least one note on the first beat- jack_midi_dump will show this note plays but not subsequent notes (on 1st playback).

This is pretty much the last MIDI bug standing far as I'm aware - certainly the most visible and 'annoying' as in recent revisions its still a case of create track, push play, stop and rewind because you're wasting your time then hit play again to hear more than just the first note of your track.

Get this sorted and I think we're good for an alpha (beta?) release as far as MIDI is concerned.

Should be fixed in SVN.
