DescriptionWhen punching in on a MIDI track in the middle of existing notes, the note-offs of those notes are never sent.

I've examined missing note off problem and contributed to this issue:
http://tracker.ardour.org/view.php?id=6882
It seems a "low level" problem: midi note bookkeeping is leaking.
